This list is Notifications to Be Made after the death of a spouse or significant other.

This list was last updated on 7/24/2009.  If it is more than six months old, verify the information.

1.  Go through your partner’s wallet, noting organizations that should be notified.  Be sure to notify any organization that could be used by an identity thief.

2.  Life Insurance Agent

3.  Banks (If Social Security direct deposits a check, the bank will automatically return any checks that arrive after death.  Do NOT change the names on your accounts until you are sure that no more checks need to be deposited.)

4.  NC Division of Motor Vehicles

“provide us with a copy of a certified death certificate”  I included a photocopy of Lew’s drivers license.

5.  Water Company

“We can start service for Water in your name but we wouldn’t be able to remove his name off the account. Please let me know if you would like a Move in to be scheduled in your name only. If you have any questions, please call us at 311 or 704-336-7600. We are available 24 hours a day and 7 days a week.”

6.  Power Company

They will check your credit report and decide if you need to make a deposit on the account.

11.  Social Security and Drug Insurance get notified by the funeral home.
